Short-Term Rehabilitation
Our rehabilitation program is designed to provide the highest level of rehabilitation and skilled nursing care to our patients in order to transition, as quickly and safely as possible, from hospital to home. Our state-of-the-art rehabilitation equipment and therapeutic modalities are the same as those used by Professional, Collegiate, and Olympic athletes to recover from injuries or surgeries.

Here at Bridgewood Post-Acute, residents can expect to be engaged in structured daily activities that are based on resident interests. Residents can enjoy scheduled outings, musical performances, fitness programs, religious meetings, gardening, and more! Calendars are given to residents to keep them informed of our daily events and all residents are encouraged to join us.
Dietary Program
Eating for Health
Here at Bridgewood Post-Acute, our Full-Time Registered Dietitian monitors weight deviations and makes recommendations accordingly. Our Full-Time Dietary Manager provides three nutritious meals to meet specific dietary and therapeutic needs, according to physician orders such as regular, diabetic, cardiac, textured-modified diets, regular nutritional assessments, weekly made-to-order breakfast club with various specialized items, and daily ethnic meal options.
Social Services
Managing all Requests
The Director of Social Services manages all requests for missing items and concerns/grievances for our facility. The Director of Social Services acts as a liaison for all members of our facility and the community by advocating for resident rights. Our long-term social worker assesses our long-term population quarterly by maintaining all ancillary services for our residents ensuring that all of their basic needs are assessed routinely and as needed to promote the highest quality of life while in our facility.
